Team Contact Form

FAQs

Who should be the “Team Contact”?

The person other teams should call right away if a game is canceled or needs to be changed. Usually that’s the team manager, sometimes the head coach. If this team were driving two hours and plans changed, who ensures the families get the message? That’s your Team Contact.

Do I submit one form or one per team?

One team per submission. Each Snowbelt team in your association needs its own form. If your association has 5 teams, you’ll submit this form five times.

Can I list multiple teams in the Notes field?

No. Do not enter multiple teams into Notes. Submit a separate form for each team.

Should I include additional contacts/coaches in the Notes field?

No. Only one contact per team is allowed. Additional contacts will be ignored.

We have multiple teams in an age level. Which team is which?

If you have 2+ teams in an age level, the “1” team is your strongest of the teams.

How do I know if you received my entry?

After clicking submit, wait for a message to appear below that button. If you have an error, please read the error and try again. If errors persist, contact the Snowbelt Scheduler. Sometimes there’s an issue and I don’t receive the response even when you see “Success,” but in those cases the Scheduler will reach out.

You need to complete this form once for each of your association's teams.
